Your Consent is REQUIRED to HOST with US: Please Review and Sign that you understand and will follow the following guidelines during the Covid -19 Pandemic. Willow Creek is committed to the health and safety of our Staff and Guests. It is commitment to our staff and our guests that we will maintain the strictest standards of sanitation and are compliant with the CDC and the State of NY, Suffolk County. A Face Mask is required for all guests with no exceptions, while you are in any area of our facility. This includes using the facilities, walking around, on the dance floor and getting up to get any type of food and or drink. You MAY NOT walk around while eating and drinking. Masks may only be removed while seated during meal service. We will if necessary remind your guests to wear their mask, and or provide a fresh clean disposable on if it is forgotten or becomes dirty /broken during their stay. Constant reminding or failure Comply with NY State Suffolk County Mandates will result in us asking your guests to leave our facility. IF in fact your guests or organization as a whole refuses to follow NY STATE Suffolk County Mandates: Willow Creek reserves the right to stop your event, and NO REFUND will be ISSUED.
