We provide a fully customizable event center with available add on fees which features a 12 x 8 Foot Stage, 4K Projector, 300 inch projector screen, 8 wireless mic set up, Interactive lighting, hookah bar, wet bar, full state of the art sound system, pool table, BBQ Grill, Full Professional Photography suite, Outside area available for smoking, foldable chairs, 6ft vendor tables, house DJ (variety of music for all age groups and ethnicities). Downstairs for Video shoots and Podcast recording. This being the case, we can help you host any event which feature show-stopping music and all kinds of live entertainment. CORONAVIRUS (COVID-19) HEALTH AND SAFETY POLICY House Of Xtreme All Host will have to fill out a pre-screening questionnaire as follows: Do you currently have any symptoms of Covid-19? Fever, cough, difficulty breathing? Have you been exposed to anyone with Covid-19? Have you traveled outside of the country within the past 14 days? All attendees will be subject to forehead or wrist thermometer readings. Temperatures 100-102 Will be re checked in 15 minutes to see if temperature decreases Temperatures over 102 will automatically be denied entry WE WILL UPDATE AND FOLLOW STRICT NYC COVID POLICY Organization Commitment This company policy includes the measures we are actively taking to mitigate the spread of Coronavirus. The team at House Of Xtreme is committed to providing a healthy and safe workplace for all employees, patrons and visitors. It is important that we all respond responsibly and transparently to these health precautions. In compliance with Health Insurance Portability and Accountability Act (HIPAA) guidelines, we are committed to treat our client’s private health information with high confidentiality and sensitivity. Scope This Coronavirus policy applies to all of our employees and visitors. Resources House Of Xtreme will make available the necessary resources (i.e. people and personal protective equipment (PPE)) in order to: Comply with all relevant CDC and safety legislation. Ensure the health and safety of employees, patrons and visitors. Policy Priorities House Of Xtreme will conduct activities and provide an environment that: Protects the health, safety and welfare of all persons, including staff, contractors and visitors. Actively encourages controlling risk. Strives for continuous improvement in Covid health and safety. The House of Xtreme will address risk management as a priority. Your health and safety is both an individual and shared responsibility of all personnel entering at The House Of Xtreme. The following responsibilities are essential to the success of the policy and are in accordance with OSHA and CDC guidelines. Responsibilities and Policy Elements Below, we outline the required actions The House Of Xtreme should take to protect employees, contractors, patrons, host and visitors from a potential Coronavirus or its variant Delta infection. Social Distancing Policy Social distancing means avoiding large gatherings and maintaining distance (approximately 6 feet or 2 meters) from others when possible (e.g. lobby, main event space, backyard and lower level). The House Of Xtreme will increase physical space between employees and patrons, visitors, contractors and host by avoiding hugging and handshakes. Services will be delivered remotely whenever possible (i.e. virtual consultation and viewings). Minimum of 2 hour gaps in booking – in an effort to reduce crowding, mixing of clients that would not normally interact and proper sanitation of ALL high traffic areas. If you are coming to view a space please have as few people with you as possible. We ask that the event planner come alone if possible. If a friend or family member is driving you, they will be asked to wait in the car. Respiratory Etiquette and Hand Hygiene Policy for Employees, Patrons, Contractors and Visitors Place alcohol-based hand sanitizers with at least 60% alcohol in multiple locations to encourage hand hygiene. Provide soap and water in the all bathrooms. If hands are visibly soiled, soap and water should be chosen over hand sanitizer. Employees must perform proper hand hygiene and wear face mask when in the event hall area. CDC’s 5 Steps of Handwashing Wet hands with clean running water (warm or cold), turn off the tap and apply soap. Lather hands by rubbing them together with soap. Lather the backs of hands, between fingers and under the nails. Scrub hands for at least 20 seconds. Hum the “Happy Birthday” song from beginning to end twice. Rinse hands under clean, running water. Dry hands using a clean paper towel or air dry them. Face mask must be worn to cover your entire nose and mouth area. If you do not have a face mask you will be asked to wait outside. We Encourage host, patrons, contractors, visitors and employees to sneeze and cough into their elbow or into a tissue. Hands must be washed with soap and water for 20 seconds immediately after coughing or sneezing. We will Provide tissue and no-touch disposal receptacles. During these trying times we ask that all payment be made via electronic transactions. Zelle, cash app, bank transfer, bank wire. Please limit cash transaction unless 100% necessary. If a Square system is used to collect payments of any kind, employees will input the patrons credit/debit card information and then have the patron insert their own card into the terminal. If a pin is not used and patrons need to physically sign and must use a pen for paperwork, we will instruct the patron to keep the pen. Routine Environmental Cleaning Policy We routinely clean all frequently touched surfaces using disinfectant wipes/solution, such as door handles, knobs, toilets, microphones, chairs, tables, music equipment, telephones, grills, vending machines, pens, receptacles, etc. Reduce Transmission Among Employees Employees who have symptoms (i.e. fever, cough, anosmia, GI symptoms, or shortness of breath) will be advised to stay home and cannot return without a negative Covid Screening. The employee will be offered a face-mask immediately to contain respiratory secretions until they are able leave the building. If an employee is confirmed to have COVID-19 infection, all employees and any host that booked an event within the last 14 days will be informed of their possible exposure to COVID-19 in the workplace but maintain confidentiality as required by the Americans with Disabilities Act (ADA). Sick employees CANNOT return to work until they have a negative COVID-19 screening after a 14 day home isolation and consultation with their HCP and state and local health departments. Employees who are well but have a sick family member at home with COVID-19 should notify House Of Xtreme Management immediately and stay home until they are cleared by their HCP and state/local health department to return to work. Personal Protective Equipment (PPE) ALL Employees MUST WEAR FACE MASK FULL NOSE AND MOUTH COVERINGS AT ALL TIMES DURING EVENTS. Patrons, contractors, host and visitors who are not vaccinated are required to bring a mask from home for their appointments. If a Patrons, contractors, host and visitors are vaccinated, mask use is optional. (Scannable Vaccination Card Is Required). Proper Mask Use: The mask should be worn all the way on so that it covers your nose and mouth. Do not wear your mask under your chin, below your nose, or in other positions such as dangling around your neck. When completely off, store the mask in a clean place. Avoid touching the front of the mask when removing it from its storage place. Do not place mask in your pocket as this increases risk for cross contamination. Avoid touching your mask to reduce contamination of the mask and your environment. If you need to touch or adjust your mask for any reason, please perform hand hygiene before and after. Remove and dispose of the mask once it is moist (e.g. from the humidity in your breath) and when the mask becomes soiled, damaged or difficult to breathe through. Additional Measures There will be no washable or reusable table wear, cups, etc. for any event. ALL ITEMS MUST BE DISPOSABLE. NO BUSINESS CARDS WILL BE GIVEN, NO PENS WILL BE REUSED.
